Students applying to Potential Christian Academy must present an immunization record (DH 680) and a current Florida School Entry Physical Examination Form (DH 3040).  In keeping with Florida law, students cannot start classes without the necessary health documentation submitted to the Admissions Office. Immunization records must remain valid throughout the entire school year. Parents can obtain immunization records, health examinations, and screenings from their child's doctor or the Broward County Health Department.

Hepatitis B
Measles-Mumps-Rubella (MMR)
Polio Series
Varicella (Chicken Pox)

Diphtheria-Tetanus-Pertussis Series

Measles-Mumps-Rubella (two doses of Measles vaccine, preferably MMR)
Polio Series
Varicella (Chicken Pox)

Hepatitis B Series
Second Dose of Measles vaccine (preferably MMR)
Tetanus- Diphtheria Booster
It is recommended that students receive a scoliosis screening at this time.
